REQUIRED STUDENT HEALTH INFORMATION

The goal of the district is to pivot to in person instruction as soon as possible so it is essential that we are prepared to allow students into our buildings without delay when that time comes.  This involves having all health services forms, immunizations, and medications in place now.

STUDENT HEALTH INVENTORY (SHI):

All schools in Washington are required to collect updated student health information each year.   Due to the current public health situation, it is more important than ever to have accurate health information available for all students so we ask you to complete the SHI with this in mind.   Here is the Annual Student Health Inventory Form to complete and submit this form.

MEDICATIONS:

For those students needing medications at school, we need this Medication Authorization form completed and signed by both health care provider and parent and turned into school or faxed to Health Services at 360-833-5575.

IMMUNIZATIONS:
